  • Credit Evaluation Form 
    • Fill out this form if you have credits from other colleges that are being transferred to WVC
  • Enrollment Verification (EV) Form 
    • Required every quarter to continue with Running Start. Fill out your sections and turn in to your high school counselor

      • Interested in Summer? Download this form and turn in to your high school counselor. For any questions about Summer, please contact Maria Christina Monroe mmonroe@wvc.edu.
  • Exit Form
    • If you are leaving the program and returning to the high school, fill out this form with your adviser
  • Graduation Application
    • Ready to graduate? Fill out this form to receive your degree!
  • Late Withdrawal Petition
    • If you are needing to withdraw from a course past the last day to withdraw due to an extenuating circumstance, fill out this form
  • Progress Report
    • If you are on probation, please send this form to your professors every 3 weeks and return the completed forms to your adviser
  • Release of Information
    • If you would like to grant permission to a trusted individual to access your WVC information, please fill this out and turn in to Running Start
  • Residency Form (in-state tuition charge)
    • If you are taking additional credits and being charged tuition, please fill this form out to ensure you are paying the in state rate
  • Spring Quarter Eligibility Adjustment Form
    • For counselors to use to assist them in calculating remaining FTE for students starting Running Start in Spring Quarter
